San Jose State University Course:
EDUC 380, Personalized Applications of Health Education for Instructors of Adult, Vocational and Career Technical Ed.
One unit of SJSU upper division course credit is awarded.

FEES
  • Three separate fees are required.
  • Course fee: $159 payable to Santa Clara LEA. 
  • Materials & Handling: $25 payable to Lee Clark.
  • Refundable Book Deposit $50 (separate check) payable to Lee Clark. We lend you the book; you do not have to purchase it separately. 
  • The book deposit is not cashed; it is held until the book is returned.
  • Registration is handled directly through our office. 
  • The textbook is provided for your use for six weeks; you do not need to purchase one.
  • When the text book is returned (in satisfactory condition), we send the check back to you along with your certificate of successful completion which is accepted by CCTC.
  • Bounced checks" subject to $25 service charge.

SEND YOUR APPLICATION

Fill in and mail to OUR OFFICE (not SJSU). 
Enclose all fees described above (three separate checks or money orders).

Upon receipt, we will send you:
  • Email instructions and receipt.
  • Textbook and study-guide/open book test by USPS Priority mail (a signature will be required). 
  • You may indicate a special mailing address such as your work in order to sign for the book.

Schedule
  • Register and start anytime. 
  • NOTE: SJSU transcripts are only available following the end of the semester. However, CCTC is familiar with our health course and accepts our certificate in lieu of transcript.
  • Complete within six weeks. (This one unit course is designed to require 15 - 20 hours to complete.)
  • When finished, mail the completed test and book back to us in bubble wrap. You will receive your grade and certificate within two weeks (or less by special request).
  • In case of not achieving a passing score, the test may be retaken for a $25 fee and a maximum grade of B-.
